What is Rehab at Home program?

After surgery or a hospital stay, you might need some time to recover and get back to feeling like yourself. Instead of staying in hospital for rehab, eligible members+ may be able to receive treatment at home, where it’s more comfortable and easier to settle into a routine, if it’s clinically appropriate.* It’s all about helping you heal in a way that works for you. ahm pays benefits towards the Rehab at Home services. If you need additional treatment, there may be out of pocket costs. 

Total Joint Replacement Rehab

Recover from a hip or knee joint replacement in the comfort and familiarity of your own home. Depending on your specific health needs, your program will typically run for 5- 6 weeks, with 1-2 visits from a physiotherapist each week.

General Rehab

Rehab treatment to support your recovery from a procedure required as a result of various conditions like heart or lung disease, stroke, injuries or fractures from falls, or cancer. Expect to receive 1-2 home visits per week for 6-8 weeks, depending on your specific health needs.

How does Rehab at Home work?

Check your cover and waiting periods

  • Must hold residential hospital cover that includes Rehabilitation as an Included or Restricted service, and have served waiting periods. 
  • Must have an upcoming hospital procedure or already be admitted in hospital and not currently receiving any inpatient rehab.

Organising your referral

  • During your hospital stay, speak with your treating medical practitioner about your rehab options and whether care at home could be appropriate for you.
  • If clinically appropriate, and available in your area,# your doctor or a member of your hospital team, including the discharge planner or physiotherapist, can help arrange a referral to the Rehab at Home Program.
  • Once your referral has been confirmed, a member of the Amplar Home Health team will contact you (or your carer) before or shortly after you leave hospital to arrange an initial assessment.

Personalised rehab plan and support

Together with any recommendations from your hospital care team, a personalised rehabilitation plan will be developed to support your recovery.

Round-the-clock support

  • While you receive your rehabilitation at home, your care team will stay in close contact to confirm your appointments, monitor your progress, and manage any issues or concerns you may have.
  • You’ll have access to ongoing support through an after-hours telephone line, staffed by health professionals, including registered nurses, who can provide information and assistance 24/7 whenever you need it.

Are you eligible for Rehab at Home?

You must hold an ahm hospital cover that includes rehabilitation as an Included or Restricted service, and have served any relevant waiting periods. Your health practitioner also needs to consider it clinically appropriate.

You must already be admitted to hospital (and not already doing in-patient rehab). Rehabilitation services will be arranged as soon as possible after you’re discharged from hospital.
